  • Providers may deny coverage based on a health problem.
  • Providers may limit coverage on a pre-existing condition by attaching an elimination rider, applying an exclusion period of up to 12 months, reviewing medical records for the 12 months prior to coverage if a claim is made in the first 12 months.
  • Pregnancy may be classified as a pre-existing condition but not genetic information.
  • Creditable coverage may be applied to a pre-existing condition exclusion period with previous coverage maintained with no lapses over 63 days.
  • The cost of premiums may be based on health status and increase with renewal.
  • Coverage may not be cancelled due to illness.
  • If a provider withdraws from the insurance market, coverage will be cancelled.
