  • For individual health insurance policies, insurance providers can deny coverage due to health issues.
  • With pre-existing conditions, the insurance company can only withhold coverage for 12 months. However, the provider may permanently exclude a pre-existing condition from coverage.
  • Qualification for creditable coverage requires maintaining continuous coverage with no break longer than 90 days.
