On February 4, 2004, Fortis, Inc. formed Assurant and merged into it. One of the changes was to move the headquarters from Nevada to Delaware. The other was to create new recognition for the company as Assurant instead of Fortis. Assurant has taken on the business operations and obligations of Fortis, Inc., operating via four segments – Solutions, Health, Employee Benefits and PreNeed.
Assurant's specialties are their small employer group health insurance for employers with 2 to 50 employees and health insurance plans for full-time college students. More than 1.1 million people across the country call Assurant their provider. RightStart Plan:This is Assurant's most affordable major medical product, offering base benefits for everyday application. Plan rates are kept low by inherent plan limitations and maximums for annual, outpatient and prescription drug benefits.
Value Plan: Assurant's Value option offers an effective solution for individuals looking to minimize their out-of-pocket costs. Plan features include no maximum for annual benefits, outpatient services, or prescription drug benefits. This product also includes a lifetime benefit of up to $6 million.
One Deductible Plan: This product streamlines your paperwork and pocketbook with a single deductible option. Families who select this product will share a single deductible, rather than managing a separate deductible for each member. The One Deductible Plan can also be applied in conjunction with a Health Savings Account (HSA). Plan members also benefit from another Assurant Health exclusive offer, a "Healthy Discount" when you renew your policy.
PPO X-tra Plan: Assurant's X-tra option presents a range of deductible options, from $500 to $5000. This premium-conscious product provides large incentives for in-network treatment, so that your costs are under your own control.
Physician Hospital Plan: This product offers the most deductible choices, as well as discounts for in-network physican visits and hospital treatments. Enjoy unlimited office visit copayments with an optional Doctor Office Copay, and reasonable out-of-pocket expenses for non-network services.
Traditional Plan: Members who adopt a Traditional Plan benefit from optimum freedom of choice and flexibility in designing their own healthcare. Choose your doctors and hospitals without price penalty, and enjoy the most flexibility available under Assurant. This product is especially ideal for individuals who reside out of an immediate in-network region.
